One clinic per specialty.

Clinic Marketing in Singapore

Singapore is 730 square kilometres with one patient pool. If an agency runs SEO for two aesthetic clinics chasing the same treatments, it is not serving both — it is deciding which one ranks. If it runs Google Ads for two urologists, it bids them up against each other and charges both for the privilege.
We do not do that, because it cannot be done honestly.

Your agency probably works with your competitors

Ask them. Most clinics never think to, and most agencies would rather the question did not come up.
It matters more here than almost anywhere else in the world, because of how small Singapore is. There is no “different city” to put the second client in. Every clinic on the island is competing for the same patients, in the same search results, in the same ad auctions.

In search, someone has to lose

There are ten organic results and three map-pack positions. When one agency represents four aesthetic clinics, it is allocating those positions between them — and it has an obvious incentive to give the best ones to whoever pays the most. That decision is never disclosed to the clinic that loses.

In paid ads, the maths is not debatable

Google prices your click based on who else is bidding. Add another advertiser for the same keywords and the clearing price rises — for everyone in it. An agency running ads for several clinics in one specialty is manufacturing its clients competition and billing both sides for it.

What we do instead

One clinic per specialty. When the slot is taken, it is taken — we will tell you honestly and recommend someone else. That costs us revenue. It is the reason the clinics we do work with get our full effort rather than a share of it.

Compliant by default, not as an afterthought

Healthcare advertising in Singapore is governed by the Healthcare Services Act, the Healthcare Services (Advertisement) Regulations, and the SMC Ethical Code and Ethical Guidelines. In practice that means:
  • No patient testimonials, and no reviews solicited or displayed in ways that function as testimonials for a medical service
  • Tight constraints on before-and-after imagery
  • No superlatives, no comparative claims, no “best” or “leading”
  • Restrictions on promotional pricing and discounting of medical services
  • Careful handling of practitioner credentials and claimed expertise
Generalist agencies get this wrong regularly — and when they do, the regulatory exposure sits with the clinic and the registered practitioner, not the agency. You carry the risk of your marketing partner mistake.
We build campaigns inside these constraints from the start rather than retrofitting them after a complaint. In practice that means education-led content, credential-led authority, and demand capture through search rather than persuasion through claims we are not allowed to make.
This page is general information about marketing practice, not legal or regulatory advice. Final responsibility for advertising compliance rests with the licensee.

What clinic marketing actually involves

Search visibility for the treatments you want more of

Most clinics rank for their own name and little else. We build treatment-level and condition-level pages that capture patients searching for the procedure, not the practice — structured to satisfy both Google medical content standards and the advertising regulations.

Google Ads that do not waste your budget

Specialist keywords in Singapore are expensive. The work is negative keywords, intent filtering, and landing pages that convert — so you are paying for prospective patients rather than students, job-seekers and competitors.

Google Business Profile and Maps

For a clinic, the map pack is often worth more than position one organic — patients search by location and act immediately. This includes review strategy built to stay within the advertising rules.

Websites that convert enquiries

Appointment-focused builds on WordPress and Shopify, fast on mobile, with enquiry paths that work for a patient deciding between three clinics.

Reporting you can actually read

Monthly reporting on enquiries and cost per enquiry — not impressions and “engagement”.

You deal with the person who does the work

Realisma has been running since 2003. It is led by Adrian Heng, with a team of ad traffic and lead conversion specialists brought in for delivery. Your business isn't treated like one of the products in a factory.  It's cared for like a work of art and curated for success.
That is deliberate. There is no account manager relaying your brief to a junior you never meet. The person you brief is the person leading the campaign — which is also why the client list is short and the one-clinic-per-specialty policy is possible at all.

Questions clinics ask

Yes. If we already work with a clinic in your specialty, we will tell you at the first conversation and, where we can, point you to someone good. We would rather lose the enquiry than compromise an existing client results.

Competing for the same patients for the same procedures in Singapore. Two aesthetic clinics both marketing injectables is a conflict. An aesthetic clinic and a paediatric practice is not. If it is borderline, we will talk it through openly before either of us commits.

Then we cannot take you, and we will say so immediately. This is why the availability table on this page exists.

Yes — within defined limits. The Healthcare Services Act and its advertisement regulations set out what is permitted. Factual information about services, practitioners and credentials is allowed; testimonials, superlatives and comparative claims are not. Most of what works for clinics is education-led content and demand capture rather than persuasion advertising.

Testimonials for medical services are restricted, and this is one of the most common places clinics get into difficulty. There is a meaningful difference between a Google review a patient leaves independently and a testimonial a clinic solicits and publishes. We will walk you through where the line sits for your specific services.  There are compliance guidelines on META and Google for health advertising and it's a minefield that can lead to ad account suspensions if you aren't experienced.

Realistically six to twelve months for meaningful movement in organic search in a competitive specialty. Google Ads produces data within weeks and optimizes by month 4 if your adspend is healthy. We normally run both so you are not waiting on SEO alone and if you aren't running both, your risk exposure to single channel failure is higher.

Yes — we work with practices in Malaysia and the UK. Different market, no conflict and wide enough geographic distance to have multiple clients in the same speciality since they don't fight for the same patient pool.

Which describes your practice?

The two work differently. Aesthetic clinics compete on visibility in a crowded treatment market; specialist practices convert a small number of high-value patients who research carefully. Pick the one that fits.

Aesthetic clinics

Injectables, lasers, skin and body. Crowded market, expensive clicks, and the tightest advertising restrictions in healthcare.

Specialist practices

Consultant-led practice, referral pathways, low search volume and very high patient value. Credentials are the differentiator.
